To make a claim simply complete the form below. If you require a third party claim form, please contact us on 01730 268592 (option 1) or by email.

Once submitted we will send a claim form via email; with sections to complete for both you and your vet.

The form has step by step instructions on what sections need to be completed and by whom and then further instruction for returning your claim form to us.

Please note:

You do not need to inform us before your pet receives treatment.
We can settle directly with your vet or yourself, the policyholder.
Once received into the office we aim to have a claim decision within ten working days.
The details provided in the form should match those on your policy. If the policy information differs from the details provided we may need to request further clarification.
